Things to bring to your Appointment:
- Driver’s license or state ID
- Insurance card / cards
- Co-pay (if applicable)
- Referral (if applicable)
- Labs/tests pertaining to office visit
- List of current medications
- Name, address and phone number of your pharmacy
*Please arrive 10 minutes prior to scheduled appointment time to fill out necessary paperwork.
